摘要:在工业化国家,老年人群退行性主动脉瓣狭窄非常普遍,且多有不良预后。瓣膜置换手术是迄今文献记载的唯一具有长期效果的疗法。然而,许多老年主动脉瓣狭窄患者手术风险较高,甚至不适合手术治疗。对这些患者而言,经导管主动脉瓣置换(TAVR)可以作为手术治疗的替代疗法。经导管主动脉置换的特点是不具有手术视野。因此,置换前及置换中影像是患者选择、置换前计划及置换中决策制定的关键。除了常规血管造影和二维超声心动图,多层CT扫描在经导管主动脉瓣置换中扮演着重要角色。在数据交互中,需要用先进的分析软件对三维CT扫描数据进行大量的后期处理。这一新疗法而言,基于复杂临床工作流程及图像分享的数据组织和存储至关重要。理想状态下,数据集成为综合的影像学资料,医院各个科室的所有医生都能够获取这些资料。这就给数据管理带来了新的挑战,它要求有能够横跨多点的复杂的IT设备,随着客户端-服务器交互及私有云技术的发展,这一成像技术越来越多得以实现。本文介绍了在经导管主动脉瓣置换的大背景下,基于患者的影像学数据急剧增多所带来的机遇与挑战。
