超声心动图引导心房介入术

Abstract

围介入超声心动图引导被认为是确保心房介入手术安全直观的可取方法,包括即时三维(RT-3D)成像的经食管超声心动图(TEE),之后被心内超声心动图(ICE)补充并部分替代,并已成为标准的引导方法。特别是在比卵圆孔未闭(PFO)关闭更具挑战性的房间隔缺损(ASD)修补术中,可能因为设备性能欠佳,严重并发症的风险仍然存在。其它的并发症则可能与间断超声心动图监测有关。图像融合和RT-3D ICE目前正处于临床试验阶段,可能会使空间定位更方便。目前,常采用二维ICE来引导经皮修补术,尤其用于房间隔缺损以及复杂的卵圆孔未闭。而深度镇静下的不间断经食管超声心动图也是一种替代方法。相反,“简单”卵圆孔未闭的修补则往往只需要对最终结果进行确认,因此,针对这些病例而言,短期超声心动图就已足够。
